Free Physicals for Sports July 14 & 15

GVMS & GVHS Families,

SAVE THE DATE!

If your student plans to play in any sport, they will be required to have a sports physical. Grand River Health will be providing FREE sports physicals. Battlement Mesa will have them on July 14th and July 15th from 4-6pm. Rifle will have them on July 17th and July 18th from 4-6pm.  Please check the flyer for more information.

Important: A parent or legal guardian must be present to sign the required CHSAA paperwork, including the medical history form. No student may participate in sports without this completed documentation.

¡RESERVA LA FECHA!

Si su estudiante planea practicar algún deporte, deberá someterse a un examen físico deportivo. Grand River Health ofrecerá exámenes físicos deportivos GRATUITOS. Battlement Mesa los realizará el 14 y 15 de julio de 16:00 a 18:00 h. Rifle los realizará el 17 y 18 de julio de 16:00 a 18:00 h. Para más información, consulte el folleto.

Importante: Un padre o tutor legal debe estar presente para firmar la documentación requerida de la CHSAA, incluido el formulario de historial médico. Ningún estudiante podrá participar en deportes sin esta documentación completa
